
Your First Visit
When we see you for your first visit, you will be welcomed and handed our New Patient forms to fill out so that we can gather pertinent information regarding your Medical History, Dental History, Insurance and Billing information. Please be sure to bring your list of medications and any Dental Insurance information you may have.
- We understand that your time is valuable and strive to stay on time from patient to patient.
- We ask that you arrive 10-15 minutes prior to your scheduled appointment in order to gather this important information.
- To help expedite this process, you can download and fill out these forms beforehand by visiting the Patient Forms page and emailing the completed forms to us!
- If you have x-rays from another office, we ask that you forward them to ebner_dds@outlook.com before your scheduled appointment so that the doctors can review them beforehand.

Typical Check-Up
A typical dental check-up appointment consists of a cleaning and a thorough examination of your teeth and the surrounding structures. During the examination, we will assess your overall gum health and then examine your teeth for any cavities, fractures or tooth wear. We provide an oral cancer screening at every check-up appointment and answer any questions or concerns you may have.
At Ebner Family Dentistry, we take the time to educate our patients, walk through any findings on x-rays together and discuss ways to keep your mouth healthy moving forward.
Cleaning and Follow-Up
The cleaning will remove any stain and build-up from the teeth and will leave your mouth feeling clean and fresh. If any additional dental appointments are needed to address areas of decay, periodontal issues, broken teeth, extractions, root canals, etc. we will schedule accordingly at your convenience.
It is our priority to take the time to provide multiple treatment options to our patients so that together, we can make an educated decision about the future of your dental care.
